Dr Chong Phui-Nah graduated with a MBBS from the National University of Singapore in 1991. Director, Family Medicine Development Division since 2008 and Director, Primary Care Academy in National Healthcare Group in Singapore since 2007. As Senior Consultant Family Physician and a clinical tutor in the National University of Singapore, Dr Chong has been teaching Family Medicine to medical undergraduates and doctors and oversees Family Medicine Training of all doctors in the National Healthcare Group Polyclinics. She has special interests in chronic respiratory diseases especially asthma.

Dr Predeebha Kannan

Dr Predeebha obtained her MBBS from NUS in 1988 and has a Masters in Public Health (NUS), 1996 and a post-graduate diploma in Higher Education from NTU in 2006. As an Assistant Director, she is currently doing administrative and policy work at NHGP Clinical Services HQ as well as being involved in training in Primary Care Academy (PCA). She has conducted training programmes for Healthcare professionals and is enthusiastic about sharing information and increasing awareness about health matters among corporate clients.

 

Dr Lee Mun Tuck

Dr Lee is a senior family physician working in Clementi Polyclinic. He has obtained his medical degree and postgraduate masters in Family Medicine.With more than 15 years of experience, he is also now a professional trainer at Primary Care Academy (PCA), where he hopes to educate the public more about healthy living and disease prevention.

Ms Beulah holds the following qualifications; Master of Nursing (Critical Care) from Sydney University of Australia' Bachelor of Nursing from Monash University of Australia' Advance Diploma in Nursing (Critical Care) from Nanyang Polytechnic Singapore' Advance Wound Management Certificate from Royal Melbourne Institute of Technology of Australia and Advance Certificate in Training and Assessment from Singapore Workforce Development Agency (WDA) and a Registered Nurse. She is also a Certified Instructor by National Resuscitation Council for Basic Cardiac Life Support (BCLS) and CPR+AED courses.


Beulah has 27 years of nursing experience in a variety of acute care setting.  Since the beginning of her nursing profession from 1981 till 2009, the major part of her career was spent in teaching, supervising and training various ranks of healthcare professionals in intensive care, medical, surgical and wound care specialty. 


Currently, Beulah holds the position of a Training Consultant in Primary Care Academy, National Healthcare Group Polyclinics (NHGP). She develops and delivers health related courses and talks to the public. Her major role and passion involves in the development of curriculum and conduct training to up-skill the healthcare professionals within NHGP

 

 

Ms Angeline Yeo

Angeline has been in nursing for over 46 years. In Year 2009 she received the HealthCare Humanity Award. She was formerly the Head Nurse of Jurong Polyclinic and is currently a Care Specialist with Primary Care Academy. She is actively involved in conducting training courses for in-house staff, public and cooperate clients. She is also responsible with audits and other related matters in the area of GP collaboration.

Dr Audrey Tan holds a medical degree from the National University of Singapore and a master's degree in health promotion/ health education from the University of Edinburgh. Accredited as a public health specialist, she has been in the field of health promotion for more than 20 years, previously with the Ministry of Health's National Health Education Department, and now in the Corporate Development Division of the National Healthcare Group. Concurrently, she is also part of the team in NHG Polyclinic's Health Promotion and Preventive Care, looking at pre-illness care and community health promotion.Her experiences range from giving talks on various healthy lifestyle topics, conducting workshops, coordinating patient education in hospitals, writing and producing patient education materials, to training health professionals like nurses, doctors, pharmacists and other allied health professionals in health promotion, health behaviour change, patient education, smoking cessation, health counselling and motivational interviewing. She is also interested in the area of health literacy.The projects she was involved in also included smoking cessation for inpatients, opportunistic health screening, self-management programmes like Stanford's chronic disease self-management programme, diabetes prevention project and workplace health intervention programmes.

Dr Matthias Toh is a Consultant Public Health Physician with the National Healthcare Group (NHG) in Singapore. He is also an Adjunct Assistant Professor with the Saw Swee Hock School of Public Health (NUHS) and Associate Program Director for National Preventive Medicine Residency Program.He has special interest in non-communicable disease prevention and control, health promotion and primary care. He has participated in the planning, implementation and evaluation of clinical programmes involving Diabetes Mellitus, Stroke, Heart Failure, Asthma and Osteoporosis across NHG hospitals and the NHG Polyclinics.Since 2005, he was one of the key clinical champions in the design and building of the first operational Chronic Disease Registry (CDMS) in the NHG and Singapore.

Ms Janie Chua

Ms Janie Chua received her bachelor’s degree in Nutrition & Dietetic from Flinders University of South Australia, Adelaide, Australia. She is an accredited dietitian and a member of the Singapore Nutritionists and Dietitians Association (SNDA).Currently a Dietitian with National Healthcare Group Polyclinics, Janie provides dietetic counselling and advice to patients in polyclinics and care management centres, as well as conducting talks to facilitate workplace health promotion programmes. She particularly enjoys caring for her patients and developed meal plans to suit their individual nutritional requirement.

 

Ms Pauline Xie

Ms Pauline Xie graduated from Flinders University of South Australia in 2002 with a Masters in Nutrition and Dietetics. Upon graduation, she spent eight years in one of the major restructured hospitals which honed her clinical skills and knowledge. She has recently joined NHG Polyclinics and is leading the team of 8 Dietitians. She is passionate that her Team of Dietitians can make a difference in health improvement and disease prevention among Singaporeans

 

Ms Wong Yuefen

Ms Ms Wong Yuefen graduated from Flinders University of South Australia with a Bachelor of Nutrition and Dietetics. Yuefen has previously worked with various groups of people, from young children, corporate clients to the elderly, and has contributed her work to the media for magazines, television and radio interviews. She is also a co-author of "First Foods", a book written locally by dietitians for parents on how to feed you child. Her special interest lies in childhood obesity. Yuefen is currently a Dietitian with the National Healthcare Group Polyclinics.  Her work involves counselling patients on nutritional matters to bring about better outcomes for the management of their chronic conditions, and conducting health talks and workshops for both the public and corporate clients.

Mr Won Tin Chiang

Mr Won Tin Chiang graduated from Deakin University (Burwood), Australia, with Bachelor in Nutrition & Dietetics. During his placement, he developed keen interest in Primary Care with the area in Community Health & Development, and chronic disease management. He was the recipient of Gold Medalist  (Applied Psychology Award) for his graduating year.


Currently Gerald is a psychologist practicing in National Healthcare Group Polyclinics and supports patients experiencing depression, anxiety, insomnia and stress. Previously he has worked with patients with cardiovascular diseases and cancer patients and supported them towards achieving better psychosocial well-being. In addition, he has worked with patients with psychiatric disorders and their caregivers  to help the patients integrate back to the community.

In her role as medication safety workgroup member, she is active in improving pharmacy professional practice including dispensing in order to prevent medication error. In addition, Jasmine also assumes her role as a mentor to pharmacy students and new staff during their job orientation at the pharmacy. Yoke Kiew is a Senior Staff Nurse with NHGP. Her duties in Ang Mo Kio Polyclinic cover a wide spectrum, from acute nursing to immunization, well women clinic and as a health counselling nurse.Yoke Kiew is a keen trainer and has been involved in conducting talks on childhood development and demonstration of cooking techniques for parents of babies going on a weaning diet. Yoke Kiew has also conducted several different workshops and given talks covering a diverse tropics from NNJ , heart diseases to Diabetes Mellitus during her last six years in NHGP.She received training in coaching skills and was a trainer in nursing skills in HMI Balestier Hospital, before joining NHGP end 2003. She received her diploma in the Management of women's health back in 2005. She is also trained in a short course in disease management and the chronic disease self management programme.
